Summary:  The OIG issued a report in February of 2021 titled  Trend Toward More Expensive Inpatient Hospital Stays in Medicare Emerged Before COVID-19 and Warrants Further Scrutiny that signaled and highlighted concern with hospital upcoding or overcoding. Hospitals increased coding and billing of DRGs at the highest level by 20% between 2014 and 2019. These DRGs billed at the highest level ultimately accounted for nearly half of all Medicare spending on inpatient hospital stays. The number of stays billed at each of the other severity levels decreased. At the same time, the average length of stay decreased for stays at the highest severity level, while the average length of all stays remained largely the same. Over half of the stays billed at the highest severity level in FY 2019—54 percent— reached that level because of just one diagnosis. The OIG recommended  CMS conduct targeted reviews of MS-DRGs and stays that are vulnerable to upcoding, as well as the hospitals that frequently bill for them CMS should conduct targeted reviews of MS-DRGs and hospital stays that are vulnerable to upcoding—i.e., those that are billed at the highest severity level—and the hospitals that frequently bill for them. The question that CDI and coding should be asking is "are we in compliance with coding and billing, are we at risk for an OIG audit with our documentation, coding, and billing" Plan on attending this webinar where you will be provided with information and lessons learned from a revenue cycle consultant's perspective working with a client that was tagged by the OIG for upcoding/overcoding that really was "Updocumentation" potentially attributable to an overly aggressive CDI program.
